California Consumer Privacy Rights Request

If you are a California resident, effective January 1, 2020, the California Consumer Privacy Act (“CCPA”) allows you to request that CREDO disclose to you the categories and/or specific pieces of personal information CREDO has collected about you in the last twelve months, as well as the categories of sources for that information, the business or commercial purposes for collecting that information, and the categories of third parties with which the information was shared. Under certain circumstances, California residents may also request deletion of personal information from our records. These are rights afforded to you by California law, and CREDO will not charge you different prices or discriminate against you if you exercise those rights.

Please be aware that, pursuant to various CCPA exceptions and in order to protect your information and our ability to provide services, there are certain specific pieces of personal information we will not provide in response to requests and certain categories of information we will not delete. Among other exceptions, CREDO will not delete any information necessary to provide our services, protect the integrity of our accounts, detect security incidents or fraud, or comply with our legal obligations.

Unfortunately, at this time, we can only make this request process available to California residents. If you are a California resident, you may fill out the information below to make a request. You may make no more than two requests in any 12 month period. (If you are a current CREDO Mobile account holder, in order to receive all of your account information, you should log in to Member Services and make your request there instead of using the form below.)

In order for us to be able to match your request with the information in our records, each piece of information you provide below must match the information we have on file for you.

For security reasons, substantive responses to your request will only be made to the contact email address we have on file for you. Depending on which request option you select, we may request that you provide additional information and submit a signed affidavit for verification purposes before processing your request. We will respond to your request or let you know if we require an extension of time within 45 days.
